PAPERWORK NEEDED TO TAKE THE FIELD ON AUGUST 1st

1 copy of birth certificate
(Proof of Original and 1 printed Copy)

2 COPIES of players Final year report card on 8.5” x 11” paper - 2023-2024 school year

ALL ELECTRONIC SIGNED DOCUMENTS ARE DUE NO LATER THAN JUNE 30th, 2025

FINAL PAYMENT FOR DELAY PAYMENT CONTRACTS MUST BE MADE ON JUNE 30th 2025!

Refund Policy:

All requests for refunds must be made, in writing, and received by May 15, 2025. All refunds will have $25.00 deducted for Administration Cost. Thank you!
